November 6 - 8, 2013

CHAMPIONSHIP FORMAT
Four-Ball Stroke Play – 36 Holes. In Four-Ball Stroke Play, two players play as partners, each playing his/her own ball. The lower score for the partners is the score for the hole. If one partner fails to complete the play of a hole, there is no penalty.

CHAMPIONSHIP FLIGHT
In the Championship Flight, players will play at scratch from the Back Tee Markers.

MAYACAMA FLIGHT
In the Mayacama Flight, amateurs will be allotted 90% of their Course Handicaps. Members are required to have an established current USGA Handicap Index to compete for net prizes in The Mayacama Flight. Maximum Course Handicap is 24 strokes for men and 36 strokes for women. Section 3-5 of USGA Handicap System will be used (players competing from different tees). The Championship Committee will be using your current USGA Handicap Index as of November 1, 2013.

TEES
•Women Amateurs shall play from the Forward Tee Markers.
•Women Professionals shall play from the Women’s Back Tee Markers.
•Men Amateurs and Professionals in the Mayacama Flight shall play from the Middle Tee Markers.
•Men Amateurs in the Championship Flight shall play from the Back Tee Markers.

GOLF BALLS
Only brands of golf balls on the USGA list of conforming golf balls may be used. The list is available for your review in the Golf Shop and at www.USGA.org.

RULES OF GOLF
Play will be governed by the Rules of Golf as approved by the United States Golf Association. Questions shall be settled by the Mayacama Golf Club Championship Committee, whose decision shall be final. Knowledge of and adherence to the rules are integral parts of sportsmanship. Each contestant should place special emphasis on knowledge of the Rules of Golf. (Rule 2)

SPECIAL NOTICE
Artificial devices for the purpose of measuring distance are PROHIBITED in this Championship. (Rule 14-3)

PRIZES
The Flight Champions will receive their individual champions plaques and have their names engraved on the championship trophy. All other prizes will be in the form of gift certificates for Amateurs and cash for Professionals.

ENTRY FEE
The entry fee for this championship is $500 per two person team. The entry fee includes a complimentary practice round on Wednesday, November 6, 2013, guest fees, all championship rounds, championship prizes, and all meals and functions listed on the Schedule of Events for you and your Professional. Caddie fees are not included. Please note that it is customary for the Member to cover the cost of the Professional's Caddie Fee. The field will be limited to the first 36 teams to enter.

ENTRY CLOSING & REGISTRATION
All entrants shall apply on the official Mayacama Golf Entry Form which was mailed on September 11, 2013 or register on the Members Only Website for an electronic confirmation. All players must contact the Golf Shop to confirm receipt of entry or online registration. Verbal and telephone entries will not be accepted. Feel free to contact the Golf Shop at 707 569 2915 if you do not receive a confirmation, if you have questions about the championship, or if you need assistance logging on to the Members Only Website.

SPECIAL CHAMPIONSHIP SUPPORTER
In an effort to increase the Championship purse without significantly increasing the entry fee, we are again seeking additional Championship Supporters. A Championship Supporter's fee is $500, and these funds will be added directly to the Championship purse for distribution to Professionals and Amateurs alike. All Supporters will be recognized on the Championship Scoreboard and in the Championship Program. As a special thank you for your support, the Club will issue a certificate for a round of golf for four players (caddie fees not included), good for use on a weekday and subject to availability and Rules and Regulations of Mayacama Golf Club. This certificate has a value of $1,300. If you would like to support this event as a Championship Supporter, please check the appropriate box at registration.
